Pain Gate Theory - Physiotherapy Working Principle
The ability to feel pain is an important part of the body’s healing process. In fact, pain is the body’s way of signaling that it has been injured and requires action to heal. It is important to note that one patient’s pain is different from another’s, even if they have suffered similar injuries. Essentially, the perception of pain is a subjective experience, influenced by a complex interplay of biological, psychological, and social factors.
Although inflammation and pain are important components of the healing process (acute pain), under certain circumstances they can have negative consequences. Various processes—involving the activation of peripheral and central sensory pathways through the sensitization of peripheral nociceptors, changes in dorsal horn neurons of the spinal cord, and effects on regions of the central nervous system—can trigger a pathological sequence of events that ultimately leads to chronic pain.
In 1965, Patrick David Wall (1925–2001) and Ronald Melzack proposed the first theories that explained suffering from the perspective of the relationship between mind and body. This hypothesis is known as the ‘Gate Control Hypothesis.” According to this theory, signals to the brain first pass through three stages in the spinal cord. These include cells within the substantia gelatinosa of the vertebrate dorsal horn, nerve fibers on the dorsal surface, and transmitting cells in the dorsal horn. The gelatinous substance acts as a “Gateway” for messages sent to the brain by modulating the signals passing through it. The pain a person experiences is caused by complex interactions between these three parts of the spinal cord.
In fact, when that ‘Gate’ is closed, the brain cannot receive information from other parts of the body, including the nervous system. However, when the signals sent to the body reach a certain energy level, that ‘Gate’ opens. When a door is opened, the air flows into the lungs and causes something there, causing pain. Although the aforementioned definition included painful cognitive disorders, the "gateway theory"—as referred to above—was one of the first to suggest that cognitive disorders could also cause pain.
In their earlier studies, Melzak and Wall have proposed that there is another mechanism that regulates subcortical axons in addition to axonal regulation. In recent years, researchers have proposed that these motor regulatory regions mediate cognitive and emotional responses to pain. Ongoing studies also indicate that the intensity of signals transmitted to the brain increases when mental health deteriorates. For instance, in a person suffering from depression, the "gate" (or pathway) for signal transmission often remains constantly open; consequently, a large volume of signals reaches the brain, increasing the likelihood of experiencing pain even in response to ordinary stimuli.Furthermore, there is evidence that unhealthy lifestyle habits create an “Open Gate” state, which then results in stimulation-irrelevant pain. The “Gate Control Hypothesis” stands out as one of the most important discoveries in the history of pain research.
How the Pain Gate Works

2. Gate Control Mechanism: This "gate" is located in the part of the spinal cord known as the "horns." It can be influenced by both physical and psychological factors:

Physical factors: Non-painful stimuli, such as touch, can activate A-beta neurons; these neurons block pain signals originating from A-delta and C neurons, effectively "closing the gate." A-beta neurons possess a protective sheath (myelin) and are long, allowing them to transmit signals faster than shorter neurons that lack this protective coating.

Psychological Factors: Your thoughts and feelings help you cope with the pain. For example, “stress and anxiety open the gates” and increase the perception of pain; Additionally, relaxing and thinking positively can “close the door” and reduce stress.
3. Central Control: The brain also plays an important role in this process. It can send signals to the spinal cord to adjust the threshold, increasing or decreasing pain.
Physical therapists use pain gateway theory to treat acute musculoskeletal injuries, chronic arthritis, neuropathy, and postoperative pain. By introducing non-painful stimuli—such as touch, heat, or pressure—practitioners intentionally close "the gate" in the spinal cord. This temporarily reduces the patient’s pain, making it easier for them to perform basic corrective exercises. Physical therapists focus on the pain gate mechanism because of its clinical features:
